We are pleased to announce the schedule of "On-site Application Support" and "On-site Application Acceptance" for your My Number Card for the month of December.
Please apply for it when you shop or take a walk.

▼ What you can do at the on-site application window
(1)On-site application support
Taking your face photo （ Free ） including No appointment is required, and you can complete the application process without any need for an appointment. My number card can be picked up at the counter of Citizen's Division of City Hall.
(2)On-site application reception
In addition to application support, you can bring the required documents for identification verification procedures. You can receive your My Number Card by mail

▼ Eligibility for acceptance
If you are registered as a resident in Ichihara City and meet any of the following criteria
(1)If you are applying for your My Number Card for the first time
(2)If you already My number card holder who is a minor at the time of initial application and who is applying for reissuance of the card with an expiration date of 3 months or less
(3))Those who already have a My number card and are applying for reissuance because there is no extra space for entry

▼ What to bring (1)On-site application support
Nothing to bring when applying.
*When you receive your card, we will check your face ＆ and identity documents.


Please bring one of the following （ （ A ） or （ B ） ） identification documents.
（ A ） ： (1) "Notification card" + (2) "Photo issued by public office" （ Example ： Driver's license, driving record certificate, passport, disability certificate, residence card etc ） 1 item
（ B ） :Notice card" + "Name ・ Address" or "Name ・ Date of birth" and which the mayor deems appropriate （ e.g. ： Health insurance card, nursing care insurance card, medical care recipient card, pension notebook, pension certificate, student ID card, etc 2 items

In both cases, a portrait photo will be taken at the site free of charge.

*Citizens Division and Branch Offices at City Hall are always available （ Citizens Division at City Hall is available on 2nd City Hall Citizen's Division accepts applications on the 2nd and 4th Sundays ） and also free of charge for taking a photo of your face.
